The Monroe Center HEALTHY AGING ASSISTANCE is committed to maintaining the independence and dignity of seniors. Through services that assist seniors and their families, we find solutions for daily living needs. We currently offer outreach services in north and south county at various locations, and have just begun outreach in east Monroe.  We work closely with an alliance of county services, and use this the referral network we've researched for your needs.  These helps may include:

  • Arranging needed services so individuals are allowed to age in place
  • Partnering with other agencies for best possible information & referral
  • Arranging for transportation needs
  • Finding food assistance services
  • Offering services to homebound persons
  • Assistance with insurance reviews and processing forms
  • Providing consultation and referrals for services and options regarding living arrangements, health, and long term needs
  • Offering guidance through Social Security, Medicare, insurance and prescription plans and applications
  • Checking on safety and treatment in senior care facilities
  • Reviewing mail and bills
  • Connecting with family and assisting during emergencies
  • Filing tax credits
  • Providing case management/supports coordination
  • Advocating for desired services and liaison with other organizations
  • Notarizing documents
  • Referring home care services
  • Coordinating visiting nurse help
  • Obtaining medical equipment or supplies
  • Assessing and finding resources for home modification needs
  • Medicare Part D enrollment and assistance

    All services are completely confidential, some may be fee-based, and donations are always accepted. If you or someone you care for needs services, call our HEALTHY AGING ALLIANCE to assist you in finding solutions to problems, concerns, or confusions. In practice for over 25 years, we have the experience and knowledge of all local and other assistance. Our staff is managed by licensed social workers.

    Our Healthy Aging Alliance is beginning a new caregiver initiative. By visiting caregivers in their own home, we can help address caregiver needs by looking at the whole picture and assessing what parts of the burden might be diverted. We can suggest resources to assist the caregiver and be an advocate for services or help you are entitled to. And all of this can happen at the caregiver’s home.

    We also have Sunshine volunteers who visit and call homebound or those in nursing homes or ill, bereaved, or hospitalized members and others.
